ABSTRACT

Abstract Objective To evaluate elbow flexion in children with obstetric brachial plexus paralysis submitted to Oberlin transfer. Methods Retrospective study with 11 patients affected by paralysis due to labor who did not present spontaneous recovery from elbow flexion until 12 months of life, operated between 2010 and 2018. Results The children were operated between 5 and 12 months of life, with a mean of 7.9 months, and the mean follow-up time was 133.2 months, ranging from 37 to 238 months. Six patients (54.5%) presented a degree of muscle strength ≥ 3, measured by the strength scale of the Medical Research Council (MRC) and, according to the active movement scale (AMS), 5 patients obtained a score of ≥ 5. A negative correlation was identified between the AMS and the Narakas classification (r = -0.509), as well as between the strength scale (MRC) and the Narakas classification (r = -0.495). A strong positive correlation was observed (r = 0.935) between the AMS and the MRC demonstrating that the higher the score on the movement scale, the higher the score on the muscle strength scale. Conclusion The Oberlin surgery is a possible option for recovery of elbow flexion in children with neonatal plexopathy, demonstrating, however, very heterogeneous results, even in the long-term follow-up.


Resumo Objetivo Avaliar a flexão do cotovelo em crianças portadoras de paralisia obstétrica do plexo braquial submetidas à transferência de Oberlin. Métodos Estudo retrospectivo com 11 pacientes acometidos por paralisia decorrente do trabalho de parto e que não apresentaram recuperação espontânea da flexão do cotovelo até os 12 meses de vida, operados entre 2010 e 2018. Resultados As crianças foram operadas entre os 5 e 12 meses de vida, com média de 7,9 meses e o tempo médio de seguimento foi de 133,2 meses, variando de 37 a 238 meses. Seis pacientes (54,5%) apresentaram grau de força muscular ≥ 3, medido pela escala de força do Medical Research Council (MRC), e, pela escala de movimentação ativa (Active Momement Scale [AMS]), 5 pacientes obtiveram pontuação ≥ 5. Foi identificada correlação negativa entre a AMS e a classificação de Narakas (r = -0,509), bem como entre a MRC e a classificação de Narakas (r = -0,495). Já entre a AMS e a MRC, foi observada forte correlação positiva (r = 0,935), demonstrando que quanto maior a pontuação na escala de movimento, maior será a pontuação na escala de força muscular. Conclusão A cirurgia de Oberlin apresenta-se como uma opção possível para a recuperação da flexão do cotovelo em crianças com plexopatia neonatal; no entanto, demonstra resultados bastante heterogêneos, mesmo no seguimento a longo prazo.

ABSTRACT

Abstract Obstetric brachial plexus palsy is a rather common injury in newborns, caused by traction to the brachial plexus during labor. In this context, with the present systematic review, we aimed to explore the use of nerve graft and nerve transfer as procedures to improve elbow flexion in children with obstetric palsy. For the present review, we followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We searched the MEDLINE, EMBASE, LILACS, The Cochrane Central Register of Controlled Trials, Web of Science, Wholis and SCOPUS databases. Predetermined criteria defined the following requirements for inclusion of a study: Clinical trials, quasi-experiments, and cohort studies that performed nerve graft and nerve transfer in children (≤ 3 years old) with diagnosis of obstetric palsy. The risk of bias in nonrandomized studies of interventions assessment tool was used for nonrandomized studies. Out of seven studies that used both procedures, three of them compared the procedures of nerve graft with nerve transfer, and the other four combined them as a reconstructive method for children with obstetric palsy. According to the Medical Research Council grading system, both methods improved equally elbow flexion in the children. Overall, our results showed that both techniques of nerve graft and nerve transfer are equally good options for nerve reconstruction in cases of obstetric palsy. More studies approaching nerve reconstruction techniques in obstetric palsy should be made, preferably randomized clinical trials, to validate the results of the present systematic review.


Resumo A paralisia obstétrica do plexo braquial é uma lesão bastante comum em neonatos, sendo causada pela tração do plexo braquial durante o trabalho de parto. A presente revisão sistemática tem como objetivo exploraro uso de enxertose transferências de nervo como procedimentos para melhora da flexão do cotovelo em crianças com paralisia obstétrica. A presente revisão sistemática seguiu as diretrizes Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA, na sigla em inglês) e foi baseada em pesquisa nos bancos de dados MEDLINE, EMBASE, LILACS, The Cochrane Central Register of Controlled Trials, Web of Science, Wholis e SCOPUS. De acordo com os critérios pré-determinados, os artigos incluídos eram ensaios clínicos, quase-experimentos, e estudos de coortes sobre enxertos e transferências de nervos em crianças (de até 3 anos de idade) com diagnóstico de paralisia obstétrica. A ferramenta de avaliação Risk of Bias in Non-Randomized Studies of Interventions foi usada em estudos não randomizados. Sete estudos utilizaram os dois procedimentos; três deles compararam os procedimentos de enxerto e transferência de nervo, enquanto os outros quatro os combinaram como método reconstrutivo em crianças com paralisia obstétrica. Segundo o sistema de classificação do Medical Research Council, os dois métodos melhoraram a flexão do cotovelo das crianças de maneira similar. De modo geral, nossos resultados mostraram que o enxerto de nervo e a transferência de nervo são opções igualmente boas para a reconstrução nervosa em casos de paralisia obstétrica. Mais estudos sobre as técnicas de reconstrução nervosa na paralisia obstétrica devem ser realizados, de preferência ensaios clínicos randomizados, para validação dos resultados dessa revisão sistemática.

Abstract Evaluate the results of a series of 28 cases of high obstetric paralysis treated with the Sever-L'Episcopo technique modified by Hoffer, between 2003 and 2016. Children (mean age, four years and seven months) with adduction contracture and internal rotation of the shoulder without secondary bone deformities (Mallet class II) underwent lengthening of the pectoralis major muscle and tenotomy of the subscapularis muscle associated with transfer of the latissimus dorsi and teres major muscle to the infraspinatus muscle, moving to the function of external rotators and elevators. The mean follow-up was three years and 10 months. At the end of the study, 24 patients achieved excellent functional assessment scores, mainly of the abduction and external rotation, passing from Mallet class II to class IV. Four patients still demonstrated some degree of global movement limitation, passing from class II to class III. Regardless of the final functional gain, all patients were able to perform tasks that were previously difficult. The data from this study suggest that Hoffer's surgery is an effective method in the treatment of the sequelae of high obstetric paralysis without secondary bone deformities.


Resumo Avaliar os resultados de uma série de 28 casos de paralisia obstétrica alta tratadas com a técnica Sever-L'Episcopo modificada por Hoffer, entre 2003 e 2016. As crianças (idade média, quatro anos e sete meses) com contratura em adução e rotação interna do ombro sem deformidades ósseas secundárias (Mallet classe II) foram submetidas ao alongamento do músculo peitoral e tenotomia do músculo subescapular associada à transferência do latissimus dorsi e músculo redondo maior para o músculo infraespinhal, movendo-se para a função de rotadores externos e elevadores. O seguimento médio foi de 3 anos e 10 meses. Ao final do estudo, 24 pacientes obtiveram excelentes escores de avaliação funcional, principalmente de abdução e rotação externa, passando de Mallet classe II para classe IV. Quatro pacientes ainda demonstraram algum grau de limitação de movimento global, passando da classe II para a classe III. Independentemente do ganho funcional final, todos os pacientes foram capazes de realizar tarefas que antes eram difíceis. Os dados deste estudo sugerem que a cirurgia de Hoffer é um método eficaz no tratamento das sequelas de paralisia obstétrica alta sem deformidades ósseas secundárias.

Abstract Obstetric palsy is classically defined as the brachial plexus injury due to shoulder dystocia or to maneuvers performed on difficult childbirths. In the last 2 decades, several studies have shown that half of the cases of Obstetric palsy are not associated with shoulder dystocia and have raised other possible etiologies for Obstetric palsy. The purpose of the present study is to collect data from literature reviews, classic articles, sentries, and evidence-based medicine to better understand the events involved in the occurrence of Obstetric palsy. A literature review was conducted in the search engine PubMed (MeSH - Medical Subject Headings) with the following keywords: shoulder dystocia and obstetric palsy, completely open, boundless regarding language or date. Later, the inclusion criterion was defined as revisions. A total of 21 review articles associated with the themes described were found until March 8, 2018. Faced with the best available evidence to date, it is well-demonstrated that Obstetric palsy occurs in uncomplicated deliveries and in cesarean deliveries, and there are multiple factors that can cause it, relativizing the responsibility of obstetricians, nurses, and midwives. The present study aims to break the paradigms that associate Obstetric palsy compulsorily with shoulder dystocia, and that its occurrence necessarily implies negligence, malpractice or recklessness of the team involved.


Resumo A paralisia obstétrica é classicamente definida como a lesão do plexo braquial decorrente da distócia de ombros ou das manobras executadas no parto difícil. Nas 2 últimas décadas, vários estudos comprovaram que metade dos casos de paralisia obstétrica não estão associados à distócia de ombros e levantaram outras possíveis etiologias para a paralisia obstétrica. O objetivo do presente trabalho é colher dados da literatura de revisão, artigos clássicos, sentinelas e da medicina baseada em evidências para compreender melhor os eventos envolvidos na ocorrência de paralisia obstétrica. Foi realizada uma revisão da literatura no motor de busca da PubMed (MeSH - Medical Subject Headings) com as seguintes palavras-chave: shoulder dystocia and obstetric palsy, completamente aberto, sem limites de língua ou data. Posteriormente, definimos como critério de inclusão artigos de revisão. Encontramos 21 artigos de revisão com associação dos temas descritos até 8 de março de 2018. Frente às melhores evidências existentes até o momento, está bem demonstrado que a paralisia obstétrica ocorre em partos não complicados e em partos cesáreos, e são múltiplos os fatores que podem causá-la, relativizando a responsabilidade de médicos obstetras, enfermeiras e parteiras. Procuramos, com o presente estudo, quebrar os paradigmas de que paralisia obstétrica se associa obrigatoriamente à distócia de ombros e que a sua ocorrência necessariamente implica em negligência, imperícia ou imprudência da equipe envolvida.

ABSTRACT Objective To study the anatomy of the brachial plexus in fetuses and to evaluate differences in morphology during evolution, or to find anatomical situations that can be identified as the cause of obstetric paralysis. Methods Nine fetuses (12 to 30 weeks of gestation) stored in formalin were used. The supraclavicular and infraclavicular parts of the brachial plexus were dissected. Results In its early course, the brachial plexus had a cord-like shape when it passed through the scalene hiatus. Origin of the phrenic nerve in the brachial plexus was observed in only one fetus. In the deep infraclavicular and retropectoralis minor spaces, the nerve fibers of the brachial plexus were distributed in the axilla and medial bicipital groove, where they formed the nerve endings. Conclusion The brachial plexus of human fetuses presents variations and relations with anatomical structures that must be considered during clinical and surgical procedures for neonatal paralysis of the upper limbs.


RESUMO Objetivo Estudar a anatomia do plexo braquial em fetos e avaliar diferenças de morfologia durante a evolução, ou encontrar situações anatômicas que possam ser apontadas como causa de paralisias obstétricas. Métodos Foram utilizados nove fetos formolizados entre 12 a 30 semanas de gestação e submetidos à dissecação supra e infraclavicular do plexo braquial. Resultados O plexo braquial inicialmente tem formato de cordão durante sua passagem pelo hiato dos escalenos e em apenas um feto foi observada a origem do nervo frênico por meio do plexo braquial. Na região infraclavicular profunda e retropeitoral menor, os fascículos do plexo braquial se distribuíam na axila e sulco bicipital medial para a formação dos nervos terminais. Conclusão O plexo braquial de fetos humanos apresenta variações e relações com estruturas anatômicas que devem ser consideradas durante os procedimentos clínicos e cirúrgicos das paralisias neonatais do membro superior.

ABSTRACT Objective: To evaluate the arc of forearm pronosupination of patients with sequelae of birth paralysis and correlate with these variables. Methods: 32 children aged between 4 and 14 years with total or partial lesions of the , brachial plexus were evaluated; measurements of pronation and supination, active and passive, were made, both on the injured side and the unaffected side. Results: A statistically significant difference was observed between the injured side and the normal side, but there was no difference between the groups regarding age or type of injury. Conclusion: The age and type of injury did not impact on the limitation of the forearm pronosupination in children with sequelae of birth paralysis.


RESUMO Objetivo: Avaliar o arco de pronossupinação do antebraço dos pacientes com sequela de paralisia obstétrica do plexo braquial e correlacionar com essas variáveis. Métodos: Foram avaliadas 32 crianças entre 4 e 14 anos, com lesões totais ou parciais do plexo braquial, foram tiradas as medidas de pronação e supinação, ativa e passiva, tanto do lado lesionado quanto do lado não afetado. Resultados: Observou-se diferença estatisticamente significativa entre o lado lesionado e o lado normal, porém não houve diferença entre os grupos por faixas etárias, nem quanto ao tipo de lesão. Conclusão: Os fatores idade e tipo de lesão não tiveram efeito sobre a pronossupinação nas crianças portadoras de sequela de paralisia obstétrica do plexo braquial.

OBJECTIVE: Description of a new surgical technique for treating the shoulders of patients with sequelae of obstetric paralysis. Preliminary analysis on the results obtained from this technique. METHODS: Five consecutive patients underwent the proposed surgical procedure, consisting of arthroscopic anterior joint release followed by transfer of the latissimus dorsi tendon (elongated and reinforced with a homologous tendon graft) to the posterosuperior portion of the greater tubercle, using a single deltopectoral approach. All the patients were reevaluated after a minimum postoperative period of twelve months. The functional assessment was based on the range of motion and the modified Mallet classification system. Statistical analyses were not possible because of the small sample. RESULTS: Overall, passive and active lateral rotations increased, while medial rotation decreased. The other movements (elevation, capacity to place a hand in the mouth and capacity to place a hand behind the neck) had less consistent evolution. The mean modified Mallet score improved by 4.2 points (from 11.4 to 15.6). CONCLUSION: The latissimus dorsi tendon can be transferred to the posterosuperior portion of the greater tubercle through a single deltopectoral approach when elongated and reinforced with a homologous tendinous graft.


OBJETIVOS: Descrição de uma nova técnica cirúrgica para o tratamento de ombro de pacientes com sequela de paralisia obstétrica. Análise preliminar dos resultados obtidos com essa técnica. MÉTODOS: Cinco pacientes consecutivos foram submetidos ao tratamento cirúrgico proposto, que envolve a liberação articular anterior por via artroscópica, seguida da transferência do tendão do músculo grande dorsal (alongado e reforçado com enxerto tendíneo homólogo) para a porção póstero-superior do tubérculo maior, com o uso de uma única via delto-peitoral. Todos foram reavaliados após um período pós-operatório mínimo de 12 meses. A avaliação da função baseou-se na amplitude de movimento e na classificação modificada de Mallet. A pequena casuística não permitiu análises estatísticas. RESULTADOS: De forma geral, as rotações laterais passiva e ativa melhoraram, enquanto a rotação medial piorou. Os outros movimentos (elevação, capacidade de colocação da mão na boca e capacidade de colocação da mão na nuca) tiveram evolução menos consistente. A média do escore de Mallet modificado melhorou 4,2 pontos (de 11,4 para 15,6). CONCLUSÃO: O tendão do músculo grande dorsal pode ser transferido para a porção póstero-superior do tubérculo maior por meio de uma única via delto-peitoral, quando alongado e reforçado com enxerto tendíneo homólogo.

ABSTRACT

Objetivo: Evaluar los resultados preliminares en 10 casos de transferencias del nervio espinal accesorio al nervio supraescapular en parálisis obstétricas del plexo braquial. Materiales y Métodos: Entre 2010 y 2012, se realizaron 16 transferencias del nervio espinal accesorio al nervio supraescapular en parálisis obstétricas del plexo braquial. Se incluyeron 10 casos con un seguimiento mínimo de 18 meses. Se evaluó la fuerza muscular del hombro según la escala de Gilbert y se usaron escalas funcionales de Mallet y de Gilbert. Se compararon valores preoperatorios y posoperatorios, así como las diferencias entre parálisis de tipo parcial y total. Se usó la prueba de Student para valorar la significancia estadística de los datos. Resultados: El seguimiento promedio fue de 20.9 meses. Se hallaron valores medios preoperatorios de fuerza de abducción de 0,48 M, y posoperatorios de 2,70 M; los valores de rotación externa preoperatorios fueron de 0 M y, al final del seguimiento, de 2,4 M. Todos los pacientes mostraban patrones preoperatorios de tipo 1 tanto de la escala de Mallet como la de Gilbert, con valores posoperatorios promedio de 3,2 y 3,5, respectivamente. Se hallaron diferencias estadísticamente significativas entre estos valores. Conclusiones: Esta serie presenta valores preliminares con un seguimiento corto y su principal crítica es el bajo número de casos. Los resultados funcionales obtenidos coinciden con los de otros reportes, y avalan su uso en las reconstrucciones del plexo braquial que requieran aporte extraplexual.


Background: To evaluate the preliminary results of spinal accessory nerve to suprascapular nerve transfer in obstetric brachial plexus palsy. Methods: Between 2010 and 2012, 16 transfers of spinal accessory nerve to suprascapular nerve were performed in obstetric brachial plexus palsy. Ten patients with a minimum follow-up of 18 months were included. Values of muscle power were assessed according to the Gilbert scale, and functional scales of the shoulder (Mallet and Gilbert) were used. Preoperative and postoperative values, and the differences between partial and total paralysis results were compared. Student test was used for the statistical analysis. Results: The average follow-up was 20.9 months. Preoperative shoulder abduction power was 0.48 M, preoperative external rotation power was 0 M, and those values at the end of the follow-up were 2.70 M and 2.4 M, respectively. All patients had type 1 patterns of the Gilbert and Mallet scales, with mean postoperative values of 3.2 and 3.5, respectively. Statistically significant differences were found between these values. Conclusions: Limitations of this preliminary report are the short follow-up and the low number of cases. However, the functional results obtained are consistent with those from other reports, and they support the use of the spinal accessory nerve to suprascapular nerve transfer in brachial plexus reconstructions requiring an extra-plexual contribution.

Objective: To demonstrate the importance of performing early shoulder tomography in patients with obstetric brachial plexus palsy (OBPP). Methods: This series of cases retrospective study with level evidence IV was conducted by consulting 76 patient's medical records with OBPP divided into three age groups: ≤12 months, 13 to 24 months and ≥ 25 months. The patients were classified according to gender, affected side, type of paralysis according to Narakas classification, and by computed tomography, according to the Waters scale. Results: The association between the age groups with Waters classification was statistically significant (p=0,006), showing that patients in the group aged less than 12 months and in the group aged between 12 and 24 months had a relevant correlation between the physical examination and Waters > III when compared to patients from groups aged 25 months or older. Conclusion: This study shows a correlation between the findings in the physical examination and severe dysplasia on the shoulder of children under 24 months of age, justifying the early tomographic shoulder exam in order to achieve a better follow-up and a consider a more aggressive approach in the treatment of OBPP affected children. Level of Evidence IV, Case Series.

ABSTRACT

La parálisis braquial obstétrica (PBO) es la parálisis de los músculos inervados por raíces nerviosas originadas en el plexo braquial. La incidencia es de 0,6 a 2,6 cada 1.000 nacidos vivos. Los factores de riesgo más importantes son distociade hombro, macrosomía y el parto instrumental. Clásicamente la PBO ha sido considerada como consecuencia de una excesiva tracción del cuello fetal en el parto. Existen casos en los que no se identifican factores de riesgo; en ellos se plantea una posible causa prenatal. La atrofia muscular del miembro superior y signos de de nervación en la electromiografía, son indicadores de que la lesión del plexo braquial ocurrió intraútero. La mayoría de los estudios reportan elementos sugestivos de una causa prenatal de PBO, sin datos concluyentes de la misma. Se describe el caso de un recién nacido con parálisis braquial en el que se demuestran claros elementos de etiología prenatal.

ABSTRACT

Birth injuries are devastating to parents and carers alike. They carry the possibility of residual loss of function to the infant and thus the potential for litigation. The aim of this study was to determine the incidence of Erb-Duchenne's palsy and the identification of any contributing factors. A retrospective review over a five-year period, 2005-2009, was performed and an incidence of 0.94 per 1000 live births was noted. An association between both macrosomia and shoulder dystocia and the development of Erb-Duchenne palsy in the newborn was noted. The authors recommended the use of partograms and improved note documentation in the management of labour.


Las lesiones de nacimiento resultan devastadoras tanto para los padres como para los cuidadores. Ellos conllevan la posibilidad de pérdida residual de función para el infante y por ende la potencialidad de litigios. El objetivo de este estudio fue determinar la incidencia de la parálisis de Erb Duchenne y la identificación de cualquiera de los factores contribuyentes. Se llevó a cabo una revisión retrospectiva por un periodo de cinco años, 2005-2009, y se observó una incidencia de 0.94 por 1000 nacimientos vivos. Se observó una asociación entre macrosomía y distocia del hombro, por una parte, y el desarrollo de la parálisis de Erb Duchenne, por otra parte, en el recién nacido. Los autores recomendaron usar partogramas y mejorar la documentación de las notas clínicas durante el trabajo de parto.

ABSTRACT

OBJETIVO: Avaliar a função do membro superior acometido, por meio do escore de Mallet, nos pacientes com contratura em rotação medial do ombro submetidos à cirurgia de Sever-L'Episcopo modificada, e correlacionar a evolução com a idade e com o tempo de seguimento pós-operatório. MÉTODOS: Dezesseis pacientes foram avaliados, sendo comparadas as medidas pré e pós-operatórias do escore de Mallet, e correlacionada a diferença entre esses valores com a idade no momento da cirurgia e tempo de seguimento. RESULTADO: Observou-se melhora do escore de Mallet pós-operatório estatisticamente significante, enquanto a correlação da diferença dos valores do escore com a idade e o tempo de seguimento não foram estatisticamente significantes. CONCLUSÃO: A cirurgia de Sever-L'Episcopo modificada proporcionou melhora da função do membro superior, de acordo com o escore de Mallet. Não houve relação entre a idade e o tempo de seguimento com a função do membro.


OBJECTIVE: To evaluate upper-limb function by means of the Mallet score, in patients with medial rotation contracture of the shoulder who underwent the modified Sever-L'Episcopo procedure, and to correlate evolution with age and length of postoperative follow-up. METHODS: Sixteen patients were assessed by comparing the pre and postoperative Mallet scores and correlating the differences between these scores with age at the time of surgery and length of follow-up. RESULTS: A statistically significant improvement in the postoperative Mallet score was observed. The correlations of the differences in scores with age and length of follow-up were not statistically significant. CONCLUSION: The modified Sever-L'Episcopo procedure led to improved upper-limb function according to the Mallet score. Limb function did not present correlations with age or length of follow-up.

Introducción: El objetivo del estudio es evaluar los patrones morfológicos de lesión en parálisis obstétricas del plexo braquial en 27 casos. Materiales y métodos: Se evaluaron retrospectivamente los datos de las historias clínicas de 27 exploraciones microquirúrgicas realizadas durante el período 2009-2012. Se usó la clasificación clínica de Narakas. Se compararon los hallazgos operatorios con el patrón clínico de lesión. Resultados: Se observaron 18 patrones diferentes de lesión. Las parálisis totales mostraban el mayor tipo de diversidad de lesiones, en general, presentaban roturas de raíces altas con avulsiones de raíces bajas. El compromiso clínico de la raíz de C7 no siempre se correlacionó con los hallazgos quirúrgicos. Conclusión: Las parálisis obstétricas del plexo braquial muestran una gran diversidad de lesiones. La cirugía es altamente demandante, requiere el manejo de todas las estrategias de reconstrucción, según los hallazgos quirúrgicos. Nivel de Evidencia: IV

ABSTRACT

Introducción: El desbalance muscular no controlado de los rotadores del hombro en la parálisis braquial obstétrica determina una deformidad glenohumeral progresiva. Nuestro objetivo es analizar la función motora y los efectos a nivel articular luego de la liberación anterior extraarticular y la transferencia de los rotadores internos a los abductores / rotadores externos, en los niños con esta secuela. Materiales y métodos: Se evaluaron prospectivamente ocho pacientes menores de 4 años con deformidad glenohumeral de tipo IV de Waters, a quienes se les realizó liberación y transferencia del dorsal ancho y redondo mayor. Se utilizó el puntaje de Mallet modificado y la medición de movimiento, antes y después de la cirugía, con un seguimiento de 30 meses. Resultados: El puntaje de Mallet mejoró de 13 a 18 puntos. La abducción activa mejoró 48º y en todos los pacientes mejoró la rotación externa pasiva. Todos los valores morfológicos mejoraron en forma significativa, salvo en dos pacientes. Conclusiones: La liberación anterior extraarticular del hombro y la transferencia de los rotadores internos a los abductores / rotadores externos mejoran significativamente la función global del hombro y la morfología de la articulación glenohumeral, sin necesidad de realizar una liberación articular.

ABSTRACT

Introducción: Es una afección infrecuente; nuestra experiencia consiste en 92 casos tratados en 25 años. Materiales y métodos: De los 92 pacientes operados, 67 presentaron deformidades osteoarticulares y 31, deformidades de las partes blandas. La metodología fue retrospectiva clínico-funcional, analítica y descriptiva considerando las historias clínicas y la valoración personal. Para el tratamiento se utilizó el algoritmo de Bennett y Christopher. Se efectuó en ocasiones la microcirugía temprana del plexo. A los 4 años algunos pacientes pueden requerir cirugías paliativas. Resultados: Con las cirugías del plexo y paliativas, se consiguió, en las lesiones C5-C6, la recuperación hasta en el 80 por ciento de los casos. Tratándose de cuadros distintos no comparables que requieren técnicas diversas, se consideraron los resultados en un porcentual global. Conclusiones: La reparación microquirúrgica del plexo braquial, efectuada en tiempo y forma, mejora los resultados en hasta un 75 por ciento. Agrupamos las secuelas en dos grandes grupos: uno sin alteraciones en la morfología osteoarticular y uno con ellas (displasia glenoidea, retroversión de la cabeza humeral y aplanamiento de la cabeza humeral). Las técnicas originales de Sever y de Sever-L'Episcopo están proscritas. Destacamos la utilidad del procedimiento de Roper efectuado antes de los 5 años de edad. Entre las cirugías del plexo y paliativas se puede lograr, en las lesiones C5-C6, una recuperación de hasta el 80 por ciento.

Realizamos estudos de condução motora em 54 pacientes com plexopatia braquial obstétrica entre 10 e 60 dias de vida. Comparamos lado a lado as amplitudes dos potenciais de ação musculares compostos (PAMC) dos nervos axilar, musculocutâneo, radial (proximal e distal), mediano e ulnar. A relação entre a amplitude do PAMC do lado lesado sobre o lado são foi chamada de “Índice de Viabilidade Axonial” (IVA). Os pacientes foram classificados de acordo com a evolução clínica. O IVA correlacionou-se com o prognóstico, tendo apresentado especificidade superior e sensibilidade equivalente ao critério clínico de Gilbert (ausência de função bicipital aos 3 meses de vida) e, portanto, pode ser útil na indicação cirúrgica destes casos...


We performed motor nerve conduction studies in 54 patients with obstetric brachial plexopathy from 10 to 60 days of life. We compared side-to-side the compound muscle action potentials (CMAP) amplitudes from the axillary, musculocutaneous, radial (proximal and distal), median and ulnar nerves. The ratio between the CMAP amplitude of the affected limb and the healthy side was called “Viability Axonal Index” (VAI). We classified the patients according to the clinical evolution. The VAI correlated with the prognosis, with a better specificity and equivalent sensibility to the Gilbert’s clinical criterion (absence of biceps function at 3 months of age), therefore, it could be used for indication of surgery in these patients...

ABSTRACT

<p><b>OBJECTIVE</b>To identify the risk factors and related degrees associated to obstetric brachial plexus palsy(OBPP).</p><p><b>METHODS</b>A case-control study was performed. Neonatal records of thirty-one cases with OBPP and their corresponding maternal records from the Department of Gynecology and Obstetrics of eight hospitals in Shanghai city from 1988 to 2002 were reviewed. Four controls, all living in Shanghai were selected to match each case and were born within the same year at the same hospital. The control group also included 124 cases without OBPP. According to the uniformed data and tables used were from medical records and from pregnant women. Epidemiological study was carried out on both case group and control group. Variables for analyses would include: (1) race, age, height, family history, pre-pregnancy weight, body mass index at the pre-pregnancy (weight/height2) on those pregnant women as well as on parity of their mothers; (2) the process of delivery which includeing clinic pelvis evaluation, height of uterus, abdomen circumference,antepartum weight,body mass index before delivery, mode of delivery,the duration of active phase and 2nd stage of labor, shoulder dystocia; (3) on neonates: sex, gestational age, birth weight,affected limb, Apgar scores of 1 and 5 minutes, other birth trauma and resuscitation of infant. Statistical tests applied to these data would include Student's T test for continuous variables and chi2 analysis for discrete data. Risk calculation of OBPP was performed by univariable and multivariable conditional logistic regression analysis.</p><p><b>RESULTS</b>12 factors related to expsure were identified for OBPP through univariable conditional logistic regression analysis. When multivariable conditional logistic regression model at P = 0.1 was applied, four factors such as cesarean (OR = 0.060), forceps (OR = 65.237), birth weight (OR = 35.468), and pre-pregnancy body mass index (OR = 23.901) were selected.</p><p><b>CONCLUSION</b>Forceps delivery,macrosomia, and increase of pre-pregnancy body mass index (> or = 21) were risk factors of OBPP in the order of degrees to risk while cesarean seemed to serve as a protective factor.</p>
